Humphreys County Care and Rehab is a 91-bed facility offering skilled care that includes physical, speech, and occupation therapy as well as intermediate care. We recognize that each resident has a unique set of needs, so our interdisciplinary care team, including members of nursing, dietary social service activities and therapy, partner with the resident and family alike to develop a personalized plan of care. Our Restorative Care Department and Activity Department develop resident-specific programs to help resident maintain their highest level of function, and we encourage residents to choose activities, schedules, and health care consistent with their interest, assessments and plans of care – we want our residents to make choices about their life in our facility. Three Rivers Hospital is in Waverly, Tennessee (Humphreys County), and is 65 miles west of Nashville, Tennessee.
Three Rivers Hospital is a 25,734 square foot general acute and outpatient facility situated on an eight-acre campus, with a medical office building attached to the facility. The Hospital was originally opened in 1961 and was owned and operated by local physicians. Over the years the hospital has changed ownership and its name a few times.
The hospital at one point in time was named after the USS Nautilus – the world’s first nuclear powered submarine to travel underneath the Arctic ice cap to go where no man had gone before: the North Pole. The Nautilus Hospital was named in honor and recognition of Captain William R. Anderson who was a native of Humphreys County.
Recognizing the need to preserve and expand hospital services locally, community leaders rallied to form a Tennessee nonprofit corporation for the purpose of purchasing Three Rivers Hospital and making it a viable hospital for the local community. On August 1, 2008, Humphreys County Community Health Services, Inc., acquired all the assets of the for-profit and is in the process of converting Three Rivers Hospital to a nonprofit hospital.
Three Rivers Hospital is a Critical Access Hospital and is licensed for 25 acute care beds and 10 Swing Beds.
The physician staff includes Cardiology, Cardiovascular/Thoracic Services, Gastroenterology, General Surgery, Family Practice, Internal Medicine, Nephrology, Obstetrics/Gynecology, Orthopedics, Pathology, Pediatrics, Physical Medicine/Rehab/Pain Management, Podiatry, Pulmonary, Radiology and Urology.
The following services are provided at Three Rivers Hospital: 24-Hour Emergency Room, Laboratory, Medical, Pediatrics, Social Services, Clinical Dietitian Counseling, Physical Therapy, Radiology Services including CAT scanner, Mammography, Bone Densitometry, Echocardiogram and Ultrasound, Respiratory Therapy including Cardiac Holter monitoring and Pulmonary Function Testing, and Surgery (Endoscopic, Laparoscopic and Outpatient Surgery). The 24-hour Emergency Room is staffed with qualified Emergency Room physicians. The Emergency Department consists of 2,400 square feet, including two (2) trauma/treatment rooms and three (3) examination rooms.
The mission of the hospital is to continuously improve the health of the people of its community. In carrying out its mission and in being an active, caring member of the community, the Hospital serves the community in a variety of ways. One of the primary ways the Hospital serves the community is by providing charity care to populations for which it receives no compensation or receives compensation at rates significantly less than established rates from patients who receive partial charity. Seventy percent of the population served is Medicare/TennCare patients and the reimbursement received from the government for providing services is at best 101% of cost.
Three Rivers Hospital is committed to improving the health in Humphreys County by providing wellness programs, community health education programs, and other community benefit activities identified for all the stages of life. The Hospital participates in fundraising for “Relay for Life” and “The United Way”.

When an illness requires you to be seen by an out-of-town specialty physician who does not practice in Humphreys County, we ask that you consider Three Rivers Hospital when your physician orders Lab, Radiology, Physical Therapy, Holter Monitor and Respiratory Services. Three Rivers Hospital also provides Swing Beds for those who require extended days of care and rehabilitation after an acute inpatient stay either at our hospital or another hospital.
